Individuals who received health coverage through Tax Management Associates can access or request a copy of their Form 1095-C. This notice is reasonably accessible to all employees who are eligible to receive this form in accordance with IRS Notice 2025-15.

Employees may download their Form 1095-C by logging in to ADP: > Myself > Pay > Pay & Tax Statements
